Thank you for your nomination of a youth that you feel could benefit from a YMCA membership. It is our goal to break barriers and provide as much facility access as we can. Your nomination could be a pivotal moment in a youth's life. This form is to be completed by the guardian of the Youth seeking a membership. Once completed the form will be directed to the Fredericton YMCA Outreach Team and you may be contacted to provide additional information. Completing this form does not guarantee the youth will receive a youth access membership. YAP Memberships are given out on a first-come-first-served basis. Included with Youth Access Membership - Two (2) registered program per term (some fees may apply).
Here you will provide us with some information on the Youth you are nominating. All current and potential members are required to create an account on the YMCA website here.
For nominated youth 18 and under, a guardian will need to create their own account and add the youth to that account - you will be prompted to do so at the final step of creating your own account.
Upon completion of creating the account(s), a member of our staff will approve the account which will allow the Outreach team to activate the youth's membership.
Please provide us with the information for whom the nominee would consider their parent/guardian. If the Youth is 19+, please enter N/A in the responses.
Here you will provide information on the nominator. The nominator is an adult community support such as a Youth Worker, Teacher, Coach, Counsellor, etc.
Facility Information
Child Protection Policy for Aquatics Facilities: Children 11 years and under must be accompanied by an adult (16+ years) who must stay in the building for the duration class. Adults accompanying children 9 years and under must remain on the pool deck for the duration of the class. The accompanying adult must sign the child in & out of their lesson on the pool deck.
Wellness Centre: Members 12 – 15 years old must complete a Teen Weight Trainer Orientation and wear their ID card as proof of the completion of the course. The course can be booked through our Welcome Desk.
Registered Programs: Youth Access members have two (2) registered program per term included with their memberships (some fees apply). Additional registered programs will have a fee.
Facility & Membership Reminders: For all YMCA programs, children 9 years & under must be supervised by a caregiver (checkin/out policies apply). Children 12 & under not in programming must be supervised by a parent or caregiver 16 years of age or older.
Membership Terms & Conditions: YMCA of Fredericton Youth Access Memberships are activated in 6 month terms upon completion of survey provided by the Outreach team. YMCA of Fredericton memberships cards may not be loaned to another individual. It is the member's responsibility to inform the YMCA of any changes to their mailing address,email address, phone number. Members are required to present a valid membership card for identification when using YMCA facilities and/or participating programs. If for any reason members are unable to present a membership card, photo identification will be required for access. The YMCA is not responsible for damages to or destruction of property belonging to or in possession of the member nor for loss or theft of such property. Memberships may be cancelled at anytime for failure to follow the terms and conditions of membership.At its sole discretion, the YMCA of Fredericton reserves the right to change and/or eliminate equipment, programs, spaces, amenities and services.
Acknowledgement of Risk While YMCA staff and instructors make every reasonable effort to minimize exposure to known risks, the YMCA recommends that you consult with your physician before participating in any YMCA program and/or using any YMCA equipment or facilities. Iunderstand and agree that in registering for a YMCA program and/or using any YMCA fitness equipment or facilities, I may be involved in physical activity and that with any physical activity there is a risk of injury. YMCA Code of Conduct, Bullying, Harassment and Discrimination Policy The YMCA of Fredericton is committed to providing a safe and welcoming environment for all members, participants, staff and volunteers. Members are responsible for their own personal conduct and safety and are encouraged to ask any person whose behavior threatens their personal safety to refrain. At its sole discretion, the YMCA of Fredericton reserves the right to suspend or cancel a membership, without refund, for any inappropriate behavior or failure to comply with our Code of Conduct or Bullying, Harassment and Discrimination Policy.
I, the undersigned hereby consent to my child's participation in the Youth AccessMembership Program and release the YMCA and all subsidiaries from any liability that mayarise through participation in said program.
